This procedure combines two ‘lasers’ into one treatment to give far superior results than either procedure alone. In the past,IPL has been a proven method of photorejuvenation, and can safely and effectively reduce superficial  pigmentation. The limits of IPL however has been hampered by the number of treatments needed (often up to 4 treatments) and the inability to safely and consistently treat darker skin types.  Additionally, deeper pigmentation is relatively resistant to IPL/BBL treatments. With Fractionated Photo Rejuvenation we combine a more specific form of IPL know as BBL (Broad Band Light) together with a shallow resurfacing technique known as Fractionation to give far superior results then IPL alone.

BBL/ IPLtreatments address the superficial pigmentation, whilst Fractionated Laser penetrates into the deeper layers of the skin, targeting deeper pigmentation as well as stimulating collagen formation. Stimulation of collagen is necessary to improve skin tone, whilst reducing pigmentation improves texture.  Fractionation involves delivering very tiny columns of laser light into the dermal layers of the skin. The ‘ fraction’ of the skin’s surface treated is determined by your treating dermatologist, and is dependent on factors such as your skin type, the type of pigmentation and recovery time.

This procedure can be used for over all rejuvenation of your skin. Facial pigmentation including freckles, sun damage and melasma responds well to this method. Additionally we have obtained excellent results with this procedure on the neck and décolletage areas as well as the hands and forearms. Essentially both skin texture (pigmentation and roughness) as well as skin tone (laxity and wrinkling) can be addressed.  In patients who seek greater improvement in skin tone, a greater degree of ‘ fractionation’ is used to stimulate more collagen.

The procedure is a two step process. The first step is IPL/BBL treatment.

Make up is removed, the eyes are protected with eyepads and a cool gel is placed on the face. This facilitates even transmission of the broad band light source. Treatment takes approximately 10 minutes and is well tolerated by most patients.  After completion of this first step, a anaesthetic cream is then applied to the treated areas- this ensures that the skin is relatively numb prior to the Fractionation. It takes approximately 30 minutes for the cream to take effect. Fractionated laser resurfacing is then performed and on average takes between 10 to 15 minutes to perform.  A cool towel is then placed on treated areas, and an occlusive gel applied at the end.

Post procedure care is simple- we provide non irritating face washes and an occlusive gel to use twice a day for the next 72 to 96 hours. Sun avoidance is an absolute must for at least 2 weeks post procedure.

If several treatments are advised, we generally space treatments 4-6 weeks apart.

We often refer to this technique as a ‘ Weekend Resurfacing technique’ – if the procedure is conducted on a Friday, patient’s often return back to work on a Monday. 80-90% of recovery will be within 72-96 hours of the procedure. Down time (time where the skin is still healing and NO make up can be worn) is dependent on the intensity of the procedure, which is also dependent on the severity of the skin condition. Your dermatologist will assess your skin and discuss down time with you at the time prior to the procedure.

This procedure is a superficial resurfacing treatment and complications are very rare. The most common side effect profile are-

• Demarcation between treated sites and untreated sites. If this occurs, your dermatologist may recommend retreatment of areas not previously treated to blend in the surrounding areas.
• Post inflammatory Hyperpigmentation may occasionally be seen in darker skin types. It presents as darkening of the treated areas. This will settle down with skin lightening agents and sun protection.
• Blistering is seen in less than 1% of cases, and settles within 3-4 days.
• Infection is very rarely seen. Notify your dermatologist if you are prone to cold sores or come into contact with cold sores after the procedure.

Fractionated Photo Rejuvenation has combined two great treatments in succession to give compounded results. Fractioned Photo rejuvenation has also enabled me to effectively and safely address pigmentation issues in darker skin types including Asian and Latino patients. The use of IPL alone is relatively contra- indicated in darker skin types, however with fractional laser it opens up avenues once closed.

In summary this is both a safe and excellent method of treating the vast majority of pigmented skin conditions including freckles, solar pigmentation, age spots and melasma (facial pigmentation).
